Heath and Wellbeing Plan

An overview of Hart’s health and wellbeing needs, our planned actions and the priorities driving our work between 2025 and 2028

We are committed to supporting the health and wellbeing of everyone who lives, works and visits our district. While Hart is generally a healthy place to live, we know that not all residents have the same opportunities to live happy, healthy and independent lives. Our Health and Wellbeing Plan sets out how we will work with communities and partners to reduce inequalities and help people stay well for longer.

The Health and Wellbeing Plan 2025–2030 outlines the key priorities we will focus on over the next five years. These priorities are based on local data, national guidance and conversations with partners, community groups and residents.

The plan focuses on three main areas:

  • Active lives – supporting people of all ages and abilities to be more active in ways that work for them.
  • Preventing poor health – helping residents make healthy choices, reducing preventable health conditions and supporting those at higher risk.
  • Access to support and information – ensuring residents can easily find and use the support available to them, including those who are digitally excluded.

Across all areas, we are committed to reducing health inequalities, supporting strong communities and improving quality of life.
